Anopportunity exists for a Patient Access Coordinator in Population Care Management at Penn State Health in State College, Pennsylvania. As a member of a Care Management team, the Coordinator will be responsible for the daily operations and routing communications between a designated patient population to assure appropriate and timely coordination of medical service, access, physician and service referral, and direction of services offered at the organization and community. They will provide high quality scheduling experience for the designated patient population. The Coordinator will also maintain data matrix of patient interventions with to analyze patient outcomes. This is a full-time, day-shift position.
Answers inbound calls from patients and internal stakeholders, redirecting patients as needed and performing all elements of scheduling accurately and politely at point of patient contact. Places outbound calls to support scheduling
Demonstrates sound knowledge and judgment involving appropriate clinician when presented with a clinical concern from a patient
Acts as a liaison to Hershey Medical Center/ Penn State University staff and customers and responds to identified needs that support the goals and objectives of the institution relative to service excellence
Collaborates with Medical Group Practice Sites and Department leadership in the resolution of operational problems and provides assistance for health promotion activities
Adheres to all organizational policies and procedures
High school diploma or equivalent
Graduate of an accredited Medical Assisting (MA) Program or Licensed Practical Nurse (LPN) program required
Three years’ of experience as a MA or LPN in an outpatient practice or clinical specialty area
Active PA license status as a Certified MA, Registered MA or LPN
Excellent oral communication skills and computer skills
P REFERRED QUALIFICATIONS
Associates degree in Healthcare Administration or a related field
Prior Call Center, Healthcare IT, or Healthcare operational experience
Strong customer service background
