SUMMARY OF POSITION:
Integral part of the Structural Heart Team working directly with Multidisciplinary Team and their patients, as well as clinical staff from multiple departments.
Registered Nurse that is accountable for effective management and coordination of care of patients primarily being evaluated for structural heart procedures- TAVR, TMVR, MitraClip, Valvuloplasties, Paravalular leak closures, ETOH Ablation, ASD/PFO closures, and Watchman.
Involved in coordinating care for Structural Heart patients by facilitating patient evaluations and prompt scheduling. Acts a direct liaison to the patient /family on behalf of the Team. Assists in the management of patient care from introduction to the Team to follow up care post procedures.
Essential job duties include: daily triage of messaging, timely scheduling, obtaining appropriate records necessary for clinical appointments and procedures, coordination of follow up care per standards of care and registry requirements, and maintaining appropriate and required documentation for all patient care. Collects data for Registry use as needed.
Must have effective communication and interpersonal skills- comfortable talking to patients in person and on telephone. Self-driven with high degree of organization essential.
Manage high risk and high complexity cardiac conditions working closely with the Structural Heart Coordinators.
Bachelor's degree in Nursing or Master's degree in Nursing required 3 years clinical experience in specialty area required
Currently licensed to practice as a registered nurse by Pennsylvania Board of Nurse Examiners required
KNOWLEDGE, SKILLS, & ABILITIES:
Strong communications skills both oral and written required Strong organizational skills required Ability to analyze data reports on patient outcomes required
Experience as a case manager, in discharge planning or insurance issues preferred Appropriate certification for specialty area preferred Proficiency in Microsoft Office preferred Motivational interviewing skills preferred
