THIS IS A HYBRID POSITION THAT IS A MIX OF REMOTE WORK AND ON SITE WORK**
Serves as a guide helping patients traverse through the healthcare system ensuring their needs are met, overcoming obstacles that are limiting the patient’s ability to receive appropriate care and treatment. This will include arranging follow-up physician and ancillary services for patients. The navigator will be a liaison between patients and physicians. The navigator will gain expertise on community resources available to the patient and families, working in collaboration with social workers, case managers and other care team members to ensure optimal patient care.
Contributes to the achievement of established organizational goals and objectives
Adheres to the organizational philosophy of Patients’ Rights and Responsibilities, policies, procedures, quality and safety standards
Assists in coordination of follow-up medical evaluations and ancillary services, ensuring continuation of care and connection to community resources in a timely manner
Promoting patient satisfaction and quality of care by facilitating communication between the patient, family and healthcare providers
Maintains an accurate database consisting of medical and ancillary service referrals as well as percentages of completed follow-up appointments
Additional data collection as necessary
Participates in multidisciplinary quality and service improvement teams as appropriate.
Represents the department and hospital/facility in community outreach efforts as appropriate
Call patients based on health risk stratification and work with them to coordinate care
Schedule members AWV and ensure they get seen by a provider
Reviews medical records and extrapolates pertinent health information needed for determining clinical quality measures as required by various regulatory and pay for performance projects (ex: HEDIS, CMS STARS, NCQA, payors and other clinical data reporting programs).

1-3 year(s) nursing experience required
Experience and efficiency with Microsoft products
Assertive, hardworking, confident and entrepreneurial
Excellent verbal communication skills
Self-motivated, organized, reliable, sound integrity
Experience with physician– and management-level decision makers
