SUMMARY OF POSITION:

The role of the Cardiac Surgery Clinical Case Manager (CCM) includes a variety of patient care interactions and working alongside our Cardiac Surgeons and Advanced Practice Providers (APP) on a daily basis to help to coordinate our patient’s care by providing easier access to the healthcare system. The outpatient role includes patient education, coordination of patient care in the pre-operative and post-operative states, and coordination of patient care between consulting services. The CCM’s role also includes face-to-face patient care in clinic alongside the surgeon and APP as well as time in the office following the patient’s care. In addition to the above, the CCM’s role includes but not limited to: interacting with our inpatient APP team and inpatient care coordinators to coordinate discharging patient needs, discharge patient phone calls, 30 days follow-up clinic phone calls, triaging incoming patient phone calls, working with outpatient APP and surgeons regarding patient needs or issues, working with the Referral Specialist reviewing new referrals, prepping the outpatient clinic, and processing incoming paperwork and forms. Our CCM’s are a respected, integral part of the team practicing with maximum autonomy under their license.

MINIMUM QUALIFICATION(S):

Two (2) years nursing experience

Currently licensed to practice as a registered nurse by the State of employment

AHA BLS prior to end of the orientation period

RESPONSIBILITIES:

Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:

Assist with patient concerns and triage patient issues appropriately.

Learn the cardiac surgery specialty and be able to educate patients on all aspects of the specialty.

Work autonomously within the scope of practice.

Collaborate with other health care professionals and service providers to ensure optimal patient care.

Develop and maintain departmental policies, procedures, objectives, or patient care standards, based on evidence-based practice guidelines or expert opinion.

Read current literature, talk with colleagues, or participate in professional organizations or conferences to keep abreast of developments in nursing and within the cardiac surgery specialty.

PREFERRED QUALIFICATION(S):

Experience as a case manager, in discharge planning or insurance issues preferred.

Appropriate certification for specialty area preferred.

Proficiency in Microsoft Office preferred.

Motivational interviewing skills preferred.

Experience working in an outpatient setting

Experience working in an inpatient setting

Experience working with cardiac patients preferred

Experience with Care Coordination preferred
