JOB SUMMARY:

The Medication Reconciliation Technician is responsible for accurately collecting and entering current patient medications into the hospital’s information system for utilization by the physicians and providers responsible for admitting and discharging the patient from the hospital. The primary function includes patient interviews and review of internal and external systems with relevant information.

MINIMUM QUALIFICATIONS:

Bachelor's Degree in Pharmacy

PTCB Certification required

Previous retail technician experience preferred
